A surgical knee positioner is a specialized device—often referred to as a surgical leg holder—essential for stabilizing or adjusting the knee joint during orthopedic procedures. Whether you are using a White Surgical, De Mayo, or Innomed system, proper setup is critical for achieving the precise flexion or extension required for successful surgery.

1. Pre-Operative Table Preparation and Attachment

Success in the operating room starts with a stable foundation. Follow these steps to prepare your equipment:

  • Replace Standard Pads: Remove standard OR table pads and replace them with appropriate surgical pads (often green) to ensure maximum stability.
  • Secure the Device: Attach the surgical knee positioner to the OR table using a high-quality standard clamp.
  • Patient Alignment: Position the device so that it supports the leg effectively, ensuring the patient’s gluteal fold is aligned at the pad separation point.

2. Fitting the Patient and Boot Attachment

To ensure patient safety and device efficacy, the fitting must be secure yet comfortable:

  • The Aluminum Boot: Carefully wrap the moldable aluminum boot around the patient’s leg.
  • Securing with Co-band: Use co-band to wrap the boot directly to the leg.
  • Stability Loops: To prevent slipping, apply a minimum of 6 loops of co-band specifically above the ankle and around the foot.

3. Maintaining the Sterile Field and Making Adjustments

Maintaining a sterile environment is non-negotiable in the OR. Ensure your surgical knee positioner is either covered with a sterile drape or utilizes a sterile disposable boot liner.

Once the patient is prepped and draped, you can utilize the adjustment mechanism—typically a sliding block or an Alvarado style set of positions—to lock in the desired flexion or extension for the procedure.

4. Safety Precautions and Device Maintenance

Protecting the patient and the equipment ensures longevity and safety:

  • Skin Protection: Always apply protective padding under the device to mitigate the risk of pressure ulcers.
  • Routine Inspection: Perform a thorough inspection of the device for any damage before and after every surgical use.
  • Sanitization: Clean the unit using approved, non-bleach disinfectants. Manufacturer-recommended alcohol-based wipes are typically the safest choice for maintaining the material.
